If you do have TMJ problems or TMD problems or Muscle Skeletal Disorder, TMJ Diagnosis, TMD Diagnosis and MSD Diagnosis must be done before orthodontic treatment begins. Sometimes TMJ Treatment, TMD Treatment and MSD Treatment must be done before orthodontic treatment begins.

NOTICE: Dr. David Alpan is a dentist who spent two years full time in the UOP Orthodontic Specialty Training program in San Francisco to become an orthodontic specialist or orthodontist. Therefore, he does NOT do any of the procedures done by a general dentist or family dentist, such as, tooth fillings, tooth crowns, tooth veneers, dental bridges, full dentures, partial dentures, dental implants, root canal therapy, gum treatments, tooth extractions or tooth removal. He does NOT do the procedures done by a cosmetic dentist, an endodontist, a periodontist, a surgeon, or a prosthetic dentist or prosthodontist that makes full dentures, partial dentures, crowns or bridges.
The only dental appliances Dr. David Alpan uses are for straightening teeth with invisalign (clear braces), iBraces (invisible braces), Damon System braces (teeth braces). He also uses a clear plastic repositioning appliance or dental splint also called a mandibular orthopedic repositioning appliance (MORA) that fits on your lower teeth during TMJ Therapy, TMD Therapy or Muscle Skeletal Disorder Therapy to fix a bad bite before doing orthodontic treatment to permanently correct the bad bite. Neuromuscular occlusion establishes the correct bite to alleviate chronic pain, such as headaches, migraines, jaw joint pain or clicking, face and neck pain plus more than one hundred (100) other symptoms.
Dr. David Alpan does work in cooperation with all other dentists including dental surgeons when surgery may be required to improve your profile when orthodontic treatment alone will not do that. He also does Zoom bleaching or tooth whitening for which there is no charge when done after orthodontic treatment is completed for new patients, which is a savings of $500.

Do I have to fill in the insurance forms?
No, you don't have to fill in your insurance forms. When you come to our office the first time, we will ask you to sign a universal insurance claim form; we will fill in your signed claim form and send it to your insurance company when you start your treatment.
If you have local union insurance, you must bring your insurance form to our office on your first appointment. We will submit your form to your local union insurance company for pre-authorization. When we receive their authorization, we will call you to schedule an appointment to begin treatment.

Does my insurance pay for orthodontic treatment?
When you tell us the name of your insurance company and their phone number, we will call on your behalf, with your permission, to find out if you are covered for orthodontic treatment.

Does my insurance pay for MSD, TMD or TMJ Diagnosis and Treatment?
When you tell us the name of your insurance company and their phone number, we will call on your behalf, with your permission, to find out if you are covered for MSD, TMD or TMJ treatment.

How much does my insurance pay and how much will I have to pay that insurance does not cover?
When we call your insurance company, we will find out how much of your orthodontic treatment or MSD, TMD or TMJ treatment your insurance company will pay. Then we will know how much you will need to pay. Our treatment coordinator will then set up a payment schedule with you that fits your needs.

Does Medi-Cal pay for orthodontic, MSD, TMD or TMJ treatment?
Medi-Cal may possibly pay for orthodontic treatment up to the age of 18.
When you come in for your first appointment, we will use Medi-Cal's HLD form that enables us to determine if Medi-Cal would consider paying for your treatment. If you do meet their criteria, we will submit your diagnosis and treatment plan to Medi-Cal for pre-authorization.
After they examine the data we send them, they will decide to pay or not to pay for your orthodontic treatment. We will notify you of their decision and then schedule an appointment to begin treatment.
Medi-Cal does NOT pay for MSD, TMD or TMJ treatment.
